Yes, I believe you need to have an explanation at-the-ready, be be sure it doesn't come off as an excuse. I feel it is best to lead off with the proposed resolution to the issue. Problem solvers are always more valuable than excuse makers.
But never try to conceal a sizable error. The truth always comes to light. And there is also respect gained in fessing up to an error that could have been hidden. It shows strength of character.
